What do you think is going on in this ECG? Well, you see some abnormal QRS complexes with a very slow initial component. What is that component called? The delta wave. Is there a shortened PR interval? Not really. Can this be found in WPW syndrome? Yes.
Did you identify the WPW pattern on this ECG? Good. Now what type of WPW is it? This is WPW type A with an upward delta wave in V1. Remember, the whole complex in V1 doesn't have to be positive, only the delta wave. What does the type A pattern represent? Only that the bypass tract is more likely to be found in the left side of the heart. Remember the mnemonic about B and R that we went over previously on page 10-16 and in Figure 10-12? Return to that section and study the concept if you wish to review the material.
