Practice ECG 12
The first thing that strikes you about this ECG is the tachycardia. The second thing should be those pointy P waves. When you look at II, you see that they are about 3 mm high, making this a P-pulmonale pattern. Now, look at the PR interval. Short isn't it? Do you see any delta waves? No. This is LGL with tachycardia.
There are only a few additional points worth mentioning. The inverted T waves in III and aVF could represent either inferior ischemia or rate-related ischemia. The voltage in the inferior leads is also impressive, and the patient manifests changes consistent with LVH.
