We'll start you off by giving you a hint . . . this ECG is ugly! That's right, it's an LBBB. The QRS complexes are wider than 0.12 seconds and there are deep, wide S waves in V1 and monomorphic R waves in V6. The next question that should pop into your head automatically is, "Do we have any concordant leads?" The answer is yes: II, III, aVF, and V5 to V6 are all concordant. These leads represent the inferolateral areas of the heart. The possibility of ischemia exists in those areas. Always think of concordance when you see a bundle block. Does the same hold true for hemiblocks? No, hemiblocks do not have T wave abnormalities associated with them, so the rules of concordance do not apply.
